- Department ID, badge, or a recent pay stub — or a military ID, VA card, or DD-214
- A list of any current medications or supplements
- Photo ID
- Come hydrated — it makes the blood draw quicker

Common questions
Do I need to fast before my lab draw?
Not for most panels. If your provider needs a fasting draw, the clinic will tell you when they schedule you.
Does my department or command see my results?
No. NEUAGE is an independent medical practice. Your labs and treatment plan stay between you and your provider, protected under HIPAA.
Can my spouse use the program?
Yes. First responder pricing extends to spouses and immediate family. They fill out the same form and mention the connection during their consult.
How long is the first visit?
Plan on about 30 minutes for the lab draw, and a separate visit for your provider consult once results are back — usually within a few days.
